The Health and Safety at Work Act 2015 requires Te Whatu Ora Counties Manukau to ensure the safety of its employees whilst at work.

The information provided will enable Occupational Health and Safety Services (OHSS) to:

  • Make recommendations to managers on fitness to work and reasonable modifications in the workplace
  • Establish a baseline for future health monitoring and health protection

Please answer these questions honestly to ensure we can provide you with the support you may need to fulfill your role.

This questionnaire takes 5 - 15 minutes to complete.

Upon review - you may be asked to: 

  • Carry out additional tests - a written communication and a laboratory form will be sent to you by email or post
  • Make an appointment to visit with the Occupational Health and Safety Nurse or Doctor
The overall process including tests, interpretation of results and clearance takes 3-10 working days from the time you have the tests.
If follow-up is required, the process will take longer.
